Accident & Injury Form
     

All accidents, expecially those resulting in injuries, that occurr during ringette activities must be reported promptly to BCRA.

Accidents should be reported immediately, but no later than 45 days following the incident.  A signed copy of the BCRA online Accident & Injury Report Form must be received at the BCRA office before the report can be filed with our insurance comapny and any claims can be processed.  Failure to report accidents within the prescribed time or to submit an authorized signed copy of the accident report can result in unnecessary delays for the injured party should they need to file an insurance claim and may even result in disqualification of their claim.

In order to ensure our members are able to access their BCRA insurance coverage, it's important that any accidents they are involved in are reported to BCRA according to the guidelines outlined on the submission form.

If an injury occurs to any of your players, please click on the link below for the BCRA Online Accident Report Form, once you have completed the form, print off two copies (one to mail to BCRA, and give one copy to the Shuswap Ringette President).
